MyWellmont offers you personalized and secure online access to portions of your medical record. It enables you to securely manage and receive information about your health. With MyWellmont, you can:

Is there a fee to use MyWellmont?

No. MyWellmont is a free service offered to Wellmont patients.

How do I sign up?

You will be offered a MyWellmont activation code when you visit your provider. This code will enable you to log in and create your own username and password. If you have not received an activation code, please contact the MyWellmont Nurse Connection at 866-517-5873 to obtain one.

When can I see my test results in MyWellmont?

Your test results will typically be released to your MyWellmont account within 1-14 days.

Can I see all my test results in MyWellmont?

Your provider will determine the types of test results that can be accessed through MyWellmont. Certain test results of a sensitive nature will not be released to MyWellmont.

If some of my health information on MyWellmont is not correct, what should I do?

Your MyWellmont information comes directly from your electronic medical record. Contact your provider to correct any inaccurate information.

If I send a message to my provider, when can I expect a reply?

You will generally receive an answer within 2 business days. MyWellmont should not be used for urgent situations. Please contact your provider directly if a situation requires immediate attention, or dial 911 for emergencies.

Can I view a family member's health record in MyWellmont?

Yes. This is called proxy access and allows a parent or guardian to log into their personal MyWellmont account and access that family member's information. Contact the patient's provider to complete a proxy consent form to request proxy access.

Can I ask questions about a family member using MyWellmont?

You must first request proxy access to ask a question about a family member. Once you have proxy access, please be sure to access the correct patient tab to communicate with your proxy's provider.

Can my spouse and I share one MyWellmont account?

No. Due to the sensitive nature of medical information, each adult must establish their own MyWellmont account.

I forgot my user name or password. What should I do?

You may use the recovery links on the sign-in page to reset your user name or password. If you experience issues, please contact the MyWellmont Nurse Connection at 866-517-5873 to reset your user name or password.

How do I get a new activation code if I lost mine or let it expire?

You may contact the MyWellmont Nurse Connection at 866-517-5873 to receive a new activation code. For your security, your activation code will expire if not used within 30 days.

From the left menu in MyWellmont, use the Preferences section under My Account to update your personal information.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal user names, and passwords. You should control your password to ensure your account remains secure.

I was logged out of MyWellmont. What happened?

We aim to protect your privacy and security of your information. While logged into MyWellmont, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyWellmont. We recommend that you log out of your MyWellmont session if you need to leave your computer for even a short period of time.
